Palliative Care

The ProHealth Palliative Care Program provides continuity of care through a transition from the last phases of treatment for terminal illness through Home Health to the palliative focus of Hospice. This programs helps ease in patients not yet ready for the idea and reality of hospice. To ensure optimal continuity, the home care nurses who help out are actually nurses from the hospice program. Palliative care offers comfort, dignity and emphasizes quality of life through control of pain and other physical symptoms, along with relief of psychological, social and spiritual distress.

Being part of the Palliative Care program means access to hospice resources before signing up for hospice – Skilled Nurses to manage your medical condition, symptoms and quickly respond to emergencies. Social workers to help the patient or family with resources and guidance, volunteers who can provide relief and practical assistance to the family, bereavement coordinators are available for consultation. The Transition nurses are also able to quickly respond to emergencies.

  • Manage pain and symptoms such as nausea, fatigue, shortness of breath, depression and anxiety
  • Ensure patients are as comfortable and active as possible
  • Assist and support patients and families with difficult medical decisions
  • Coordinate care and treatment during all stages of illness
  • Provide 24-hour support with a registered nurse over the telephone
